Design, Validation, and Implementation of Effective Quality Control Materials for NGS/NIPS in a Clinical Laboratory

As tests for large numbers of rare genetic conditions come on-line in clinical laboratories, it is becoming increasing difficult to obtain high-quality, well-characterized human samples to use for the purposes of test development, validation, and ongoing proficiency testing. The first presentation will review how the CDC GeT-RM program is helping laboratories access samples for these purposes. Non-invasive prenatal screening (NIPS) for the common triploidies, Down (T21), Edward (T18) and Patau (T13) syndromes, as well as numerical abnormalities of the sex chromosomes, is now firmly established as an extremely valuable screening tool. However, there is currently no universally accepted or adopted proficiency test (PT) for NIPS. The second presentation will review the substantial challenges in developing PT for NIPS, and review progress in its development.

- Identify the sample resources available to laboratories through the Get-RM program.

- Explain the challenges and current progress toward developing PT samples for NIPS testing.

The course features introductory information about the state of the science as well as a series of case studies illustrating some of the most current principles and practices in the field of Molecular Pathology. The objective of this course is to increase basic and applied pathology knowledge, focusing on the molecular diagnosis of disease and to facilitate knowledge acquisition regarding issues and challenges related to care and prevention. This course is designed to meet the participants' educational needs in the physician competency area of Medical Knowledge, as defined by the Accreditation Council for Graduate Medical Education (ACGME) and the American Board of Medical Specialties (ABMS), and to support participants' lifelong learning towards a goal of promoting patient safety and improving patient care.

Contemporary hot topics in the clinical molecular diagnostics laboratory will be introduced, followed by an overview of genomic testing to build the molecular diagnostic "toolkit." Short case presentations will demonstrate practical applications of a wide range of methodologies ranging from common commercial platforms to traditional amplification methods.

    This course is designed to provide an overview of techniques and instrumentation used from the framework of the "Molecular Pathology/Molecular Diagnostic Toolkit." In this course, faculty members will present case studies to demonstrate the toolkit’s methodologies and technologies. Emphasis will be placed on molecular testing in the areas of clinical genetics, solid tumors, hematopathology, and infectious diseases. The application of novel technologies, such as next generation sequencing and informatics, will also be highlighted.

The ASCP is accredited by the ACCME to provide continuing medical education for physicians.

    The ASCP designates this online educational activity for a maximum of 3.0 AMA PRA Category 1 Credit(s)TM. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

    This continuing medical laboratory education activity is recognized by the ASCP for up to 3.0 hours of CMLE credit.     Welcome to the Fellowship-in-Service-Examination 2019 brought to you by the Molecular Genetic Pathology (MGP) Program Directors and the Association for Molecular Pathology.

    These exam questions were written by MGP faculty from many MGP-Fellowship institutions and cover the range of topics listed below. For each examination, a total of 45 questions populated from a question bank of nearly 250 questions will be randomly generated. At the end of the examination, you will receive your results along with the correct responses, an explanation of the correct choice, and references for further information. You will only receive this information for each question answered, so you are encouraged to answer all questions, regardless of whether or not you know the correct answer.

    You are welcome to take the exam as many times as you like, but please note that participants will be charged a small administrative fee ($25 for AMP members, $50 for non-members) each time they take the exam.

    Description:

    Recent advances in long-read sequencing technologies allows for a more complete identification of DNA and RNA associated with human disease. Two talks will be given to describe a few applications of these technologies.

    The first talk will present nanopore sequencing of full-length cDNA from chronic lymphocytic leukemia (CLL) with and without mutation in a splicing factor, SF3B1. SF3B1 is one of the most frequently mutated genes in and is associated with poor patient prognosis. While alternative splicing patterns caused by mutations in SF3B1 using short-read sequencing are known, identifying the full-length isoform changes may better elucidate the functional consequences of these mutations. The talk will describe a computational workflow for transcript analysis called FLAIR (Full-Length Alternative Isoform analysis of RNA), novel findings, and remaining challenges with long-read sequencing approaches for alternative splicing analysis.

    The second talk will present single-molecule real time sequencing of influenza. Infection by influenza virus can activate innate immune pathways that help control viral infection. However, these pathways are only rarely activated in single virus-infected cells. This webcast will describe how combining long-read sequencing with other single-cell techniques to determine how mutations in influenza virions affect whether infected cells activate innate immune pathways.

    Collapse Next-Generation Sequencing (NGS) Technologies for HIV Drug Resistance Testing

    Description:

    HIV-1 exists within individuals as a complex mixture of variants often referred to as a quasispecies. During the past 10 years, there have been many studies of the distribution and clinical significance of "low-abundance" or "minority" drug-resistance variants present in plasma in proportions below 20% to 30% -- the limit of detection of standard genotypic resistance testing using dideoxy-terminator "Sanger" sequencing. HIV-1 "deep" sequencing using NGS technologies have been widely used in research settings to study the clinical significance of low-abundance drug resistance mutations. In addition, NGS is commercially available for several niche clinical applications such as determining HIV-1 tropism before using a CCR5 inhibitor.

    In this webinar, Dr. Shafer will review the scenarios in which low-abundance HIV-1 drug-resistance mutations are most clinically relevant, the most influential studies of NGS for HIV-1 drug resistance testing, and the practical aspects of NGS that have so far slowed its widespread adoption for most routine HIV-1 genotypic resistance testing.
